Below you will find the information sheets, registration forms, and permission slips for the 2012 Rugby Season. They have been combined into a single PDF for easy download. Please print the file, read the information, and fill out the forms.

Remember that all players must register or renew their membership at USA Rugby and obtain a Membership ID (CIPP).

All completed forms and funds (D.R.F.C. player dues) should be turned in ASAP, but must be handed in no later than your team's 1st regular season practice. The forms and money can be handed in at the pre-season practices or can be returned to:
